Request Sponsorship from the Office of District 6

Qualifying programs and events are activities that benefit or serve District 6 residents.

Step 2.Gather your materials

Be prepared to submit:

• Information about your organization
• Justification for funding
• Executed W-9 dated within year of submittal

Step 3.Complete a sponsorship request form

Please submit this form at least 30 days before you need the funds.

Step 4.We'll review your application

We review all submissions in the order we receive them. We will contact you within 3 business days about the sponsorship opportunity.

Step 5.Receive your sponsorship funds

If we approve your sponsorship request, we will contact you to arrange payment.
